WELCOME TO FSA PREMIER

STEP 1 - Registration

To finalize your registration please fill out the form below. Note that for a successful registration all the below needs to provide.

The initial deposit, to secure your spot in one of our teams, is $300 and will be charged upon your registration using the form below.

 

STEP 2 - Documents

The following documents are required for league registration:

- Child's Birth Certificate (JPG File)

- Player's Headshot (JPG File)

- US Club Soccer Medical Release Form (download link provided with confirmation email)

You will receive a confirmation email after your registration. Please provide the documents as requested by replying to this email.
